The purpose of an Appraisal Template is to serve as the base for a single Appraisal Assessment that may run through multiple periods within a year (e.g. Multiple Quarterly Assessment that adds up to one final rating). If you have more than one assessment per year, you will need to create another separate template for it.
Step 1
To create an Apprisal Template, first, you will need to direct to the Appraisal Page by following the step below.
Step 2
Once you clicked on the menu, you will be directed to the page below, you may proceed to click the 'Add New' (5) button located at the top right corner, or click (4) to edit one of the existing templates that were created before.
Step 3
After clicking Add New (5), you will be brought to the template interface as shown below. Starting from the top (6) you will notice there are a total of 4 tabs that you will need to configure, each tab allows you to set up different elements to this Appraisal Template.
Start by choosing a name (7) for this Template/Assessment, follow by a Report Label under (8). Next under Rating (9), this is where you decide what will be the final rating scale used for this template, this can be set up here. Do take note that if you have assigned more than 2 templates/assessments to your staff at one time, the system will automatically use the default final rating scale within the system.
Step 2
Next, we will move on to the Appraiser Section, the Appraiser section allows you to define, who will the Appraiser/ Reviewer be for this assessment. Starting with (11), here you may select between three different mode
- Custom: Create an Appraiser Flow on the Spot / within this template.
- Auto: Appraiser Flow will be automatically selected based on the Appraiser Flow Template define here.
- 360: This is a unique setup to turn this into a 360-degree assessment, you may learn more here.
Step 3 (Optional)
If you have selected Auto, you may move on to the next step.
If you have selected custom, you will be able to find the additional field that appears underneath the type selection as shown below. Start by clicking on (12) to pick the first Appraiser for this assessment, a drawer will appear from the right, and you will find a list (13) of conditions or users you may select as Appraiser. Keep in mind that you may only select 1 appraiser per level.
- Level 1 Supervisor: This is the reporting person for each designated person/appraisee
- Level 2 Supervisor: Supervisor of Level 1 Supervisor
- Level 3 Supervisor: Supervisor of Level 2 Supervisor
Once selected click on Confirm (14).
After you added the first level of the appraiser, you may decide if an additional level is needed. If Yes click on (15) for an additional level, and follow the steps above. There are additional options for you to give a different role for the next level as shown in (16). You may also choose to keep someone in the loop by adding them in (17).
Keep in mind that if a level has been assigned with the Reviewer role, the subsequent role will automatically be the Reviewer.
Step 4
Now that we have set up the Appraiser, we will move on to defining the Appraisee.
Defining Appraisee allows you to set the Appraisee that will be using this Appraisal, but it does not automatically assign to them for appraisal. This is just an additional control to make sure the assessment is only available to the right group of people that you have assigned here.
Proceed to set up either with All Employee (18) or (19) for a custom group of users based on conditions.
The last option on this page is the Sign Off function (20), you may decide which role is required to sign off the Appraisal Result.
Step 5
When you are ready, you may move on to the next tab, by navigating back to the top and clicking on Timeline (21).
The purpose of this page is for you to define the time period for each activity that will be conducted within this assessment.
Starting with the Appraisal Period (22), this section allows you to set the overall assessment period, with the start & end date. If you would like to define a period that is not within the same year, you may choose to toggle Enable Next Year (23) to indicate the End Date that happens in the year after.
This section comes with an additional feature for automated reminders, you toggle it on by clicking on (24). Once toggle, you notice additional fields as shown below under (25) & (26), set accordingly and the system will automatically remind the designated user with Email & System Notification for all task that is related to this assessment.
Step 6
Next, we will set up the time period for Goal Planning (27), this setup is only required if you allow the appraisee to plan their own goals/KPI, follow by the appraiser's approval.
Do keep in mind that the due date set in this section will not prevent the user from completing the activity even if it's overdue, it only served as a record and a reminder to keep your user on track for their assigned activities.
For (28), this is where you will define, how often will the appraisee be assessed using this template. You have 4 options here, monthly, quarterly, half-yearly & annually. Each selection you made, will affect your input for the respective date field (30)
After setting the Assessment, you can choose to define a due date for the Final Rating Calibration under (31), and a specified Sign Off Due Date under (32).
Step 7
Now that we are done with the timeline, we may move to the next portion which is to design the Appraisal Form. Navigate back to the top & click on (33) to access Appraisal Form as shown below.
From the left, you will notice there are a total of four options to what you could include in the Appraisal Form.
- KPI/Goal
- Competency
- Questions
- Info
To add a section in, just left-click on any of the elements under (34), and drag them to the section on the right (35).
Under each section added, proceed to add a name to be displayed under (36) & (39), and choose a default KPI or Competency Template to be used for this section under (37) & (40). With the exception of KPI, you have an additional option (38) to decide who will have the access to add/edit/delete KPI during the planning stage.
The next type of form element is the Question (41), here you may add multiple questions that will be required for the designated to answer during the Assessment. Choose the question type by clicking on (43). If you would like the appraisee to be rated based on how they answer the questions, click on (42) to choose a rating scale option (this is based on KPI/Goal Scale Template).
The last type of form element is the Info Section (44), this section serves as an intermittent page that can be inserted in any parts of the Appraisal form, to provide additional insights or instructions to the appraisee or appraiser. There is an option for you put Youtube Video Link at (45) and a Document attachment under (46).
Step 8
Once you are done setting up the Appraisal Form, the last portion of this template to be set is the Scoring & Weightage for this Assessment, navigate back to the top & click on (47)
Right below the weightage chart, there are two options to be configured.
Appraisal Summary Display (48): this option control the display of a final summary screen that will showcase, each rating that has been given by all appraisal & reviewer, this will display their identity and rating given for each section.
Overall Appraiser Identity (49): this option allows you to control the overall display of the rater's identity for each participant of this assessment.
Step 9
After the previous 2 options, this section is where you will determine the weightage that will be given for each section under (51), and (50) allows you to decide the calculation method for each section.
Each section comes with a unique control(52), to decide what each role will be able to fill in or view during this appraisal. proceed to check this for each section that's been added to this template
Once you are ready, click on the Save (53) button that is in the right top corner.
