Creating Appraisal Template

An Appraisal Template serves as the structure for a single Appraisal Assessment. It can be used for assessments that run multiple times within a year


If you require more than one assessment in a year (e.g. Mid-Year Review and Year-End Review), you will need to create a separate template for each.


Step 1

Go to Settings (a) > Performance (b) > Appraisal Template (c).




Step 2

You will be directed to the Appraisal Template listing page.


Click Add New at the top right corner to create a new template, or click on an existing template name to edit.

There are 4 tabs to configure in this template:

Each tab controls a different part of the assessment setup.




1. General Settings

This tab consists of four sections:

  • General
  • Appraiser
  • Appraisee
  • Sign Off


A. General


Specify the following:

Note: If more than two templates/assessments are assigned to an employee at the same time, the system will automatically use the system default final rating scale.

  • Status: Active | Inactive - Set the status to Active once the template is ready to be used.



B. Appraiser


This section defines who will act as the Appraiser / Reviewer for this assessment. You may select one of the following modes:


Custom: Create the Appraiser Flow level by level directly within this template. If a level is assigned with the Reviewer role, all subsequent levels will automatically be set as Reviewer.


Auto: The Appraiser Flow will be automatically assigned based on the Appraiser Flow Template. (refer Creating Performance Appraisal Flow Template)


360: Create a 360-degree appraisal flow by selecting multiple levels of raters.


You may also toggle Enable Custom Goal Approval if you want to define a separate goal approval route specifically for this template.



C. Appraisee

This section allows you to define which employees are eligible to use this template. 


Select employees using any of the available selection methods.



Important: This does not automatically assign the appraisal to them. It only ensures that the template is available to the selected group when creating an assessment.



D. Sign Off


Tick the checkbox(es) to define which role is required to sign off on the final appraisal result.



2. Timeline

This tab allows you to define the timeline for each stage of the assessment:

  • Appraisal Period
  • Planning
  • Assessment
  • Sign Off
  • Notification


Within each section, the following tools are available:

  • Enable Next Year - When this button is toggled ON, it indicates that the appraisal will take place NEXT YEAR. Users cannot add/edit/update their tasks until then.
  • Activate Reminder - Toggle on to set reminders before or after the due date. The system will automatically send Email and System Notifications to the designated users for tasks related to this assessment.


A. Appraisal Period


Define the overall assessment period by setting:

  • Start Date
  • End Date

If the period spans across two calendar years, toggle Enable Next Year.



B. Planning


This section is only required if you allow the Appraisee to create and submit their own Goals / KPIs for approval.


Important: The due date set here does not block users from completing the activity after it is overdue. It serves as a tracking record and reminder only.



C. Assessment


Define how frequently the Appraisee will be assessed under this template:

  • Monthly (12 periods)
  • Quarterly (4 periods)
  • Half-Yearly (2 periods)
  • Annually (1 period)

Your selection will determine the number of assessment periods required to set.



D. Sign Off


Set the due date for the final sign off of this appraisal.



E. Notification


Tick the relevant checkboxes to enable:

  • System Notification
  • Email Notification

You may also define whether notifications are triggered upon assignment or completion for each role.



After defining all dates, scroll to the top of the page to view the timeline bar, which provides a visual overview of the defined periods.




3. Appraisal Form

This tab allows you to design the structure of the appraisal form.


Available elements:

  • Info
  • Goal
  • Competency
  • Question


To add a section:

  1. Drag the element from the left panel
  2. Drop it into the right workspace

To remove a section, click the three-dot icon and select Delete.



A. Info

Used to provide instructions or additional context for the assessment.

  • Section Name*
  • Section Description
  • Attachment (Upload file or insert YouTube link)



B. Competency

  • Section Name*
  • Section Description
  • Competency Template* - Select from previously created Competency Templates in PMS.



C. Goal

  • Section Name*
  • Section Description
  • Goal Library Template* - Select from previously created Goal Library Templates in PMS.

Options available:

  • Allow Progress Above 100%
  • Allow Goal Rating Score Above 100%

You may also define whether Appraiser, Appraisee, or Reviewer can:

  • Add/Edit Goal
  • Update Goal 



D. Question


Add custom question(s) to be answered during the assessment.

  • Section Name*
  • Scale (Select from Goal Rating Scale)


Then select the Question Type:

  • Text: Open-ended answer.
  • Multiple Choice: Enter question and provide answer choices.
  • Likert: Enter question and define the minimum and maximum rating descriptions. Click + to add scale levels or – to remove them.




4. Scoring & Weightage

All sections added in the Appraisal Form (except Info) will appear here.


This tab allows you to:

  • Define the weightage (%) for each section
  • Decide the calculation method
  • Configure role-based permissions



Appraisal Summary Display - Controls whether the Overall Summary screen (showing ratings from all Appraisers / Reviewers) is visible to each role. Toggle under each role to show or hide.


Overall Appraiser Identity - Controls whether the rater’s identity is visible to participants. Toggle under each role to show or hide.



Important: Any changes made will not automatically reflect. You must reset the appraisal form for the appraisee for the changes to take effect.
